All individuals participating in, performing at, or teaching at an Indiana Bellydance Collective event agree to the following:
Liability Waiver
I acknowledge that participation in any dance-related activity, such as the event for which I am now purchasing tickets (hereinafter referred to as “The Activity”), carries with it certain inherent risks that cannot be eliminated regardless of the care taken to avoid injuries. I know, understand, and appreciate these and other risks that are inherent in The Activity and hereby assert that any participation is voluntary and that I knowingly assume all such risks.
I also agree to INDEMNIFY AND HOLD HARMLESS the Indiana State Association of Middle Eastern Teachers and Dancers doing business as the Indiana Bellydance Collective and/or INBC (hereinafter referred to as “INBC”), its officers, employees, volunteers, and partners from any and all claims, actions, suits, procedures, costs, expenses, damages and liabilities, including attorney’s fees brought as a result of any involvement in The Activity and to reimburse them for any such expenses incurred.
Media Waiver
I hereby consent to be photographed and/or videotaped on behalf INBC. I consent to the use of the content of the photograph and/or videotape (including my name, performance, image, words and voice) by INBC for any purposes consistent with its mission including but not limited to: public relations efforts, social media posts, publishing content on the internet, and including content in other materials.
